Kyndryl’s Healthcare Readiness Report, released March 5, 2026, found that number after surveying health IT leaders nationwide. Three weeks later, at HIMSS 2026 in Las Vegas, that statistic didn’t land as a warning. It landed as a confession.

Health systems aren’t failing at AI because the technology is broken. They’re failing because the adoption model they inherited from traditional software procurement doesn’t apply to clinical AI. And most organizations haven’t yet confronted that mismatch in any meaningful way.

The Problem: Pilot Purgatory

The standard playbook goes like this: a vendor runs a demo, clinical leadership gets excited, procurement signs a contract, IT deploys the tool, and a 90-day pilot launches with a hand-selected cohort of enthusiastic early adopters. The pilot succeeds. Results get reported upward. A slide deck is built for the board meeting. And then nothing. The tool never reaches the next unit, the next facility, or the adjacent workflow. It lives in permanent pilot status, funded just enough to survive but never enough to scale.

This is pilot purgatory. 80% of healthcare AI projects never escape it. A separate MIT analysis found that enterprises globally are investing $30 to $40 billion in generative AI, and more than 95% are seeing no measurable ROI. Healthcare is not outperforming that benchmark.

Clinicians just talk. ROI shows up in minutes-per-encounter within the first week. No behavior change required. No custom dashboard needed to see the impact.

Every AI deployment that scales at enterprise level shares those three characteristics. It targets a specific, documented pain point. It minimizes the behavior change required to generate value on day one. And it produces ROI that’s observable using data the organization already has, within 30 to 60 days.

The inverse is equally reliable. AI tools that fail at scale try to do too much, require clinicians to adapt their workflow to the tool’s logic rather than the reverse, and deliver ROI that takes six months of analysis to surface. That’s not a value problem. That’s a design and deployment problem.

In pharmacy operations specifically, this pattern repeats with near-perfect consistency. Prior authorization AI promises a 40% reduction in intake time. It delivers that, for 60 to 90 days, while the vendor’s implementation team is still embedded. Then the team rolls off. A workflow edge case breaks the integration. Nobody internally knows who owns the fix. Pharmacists route around the tool by reverting to manual intake. Within six months, adoption is at 20%. The vendor relationship becomes adversarial. The contract gets cancelled. The AI strategy “failed.”

The tool didn’t fail. The ownership model failed.

Before any healthcare AI tool is cleared for enterprise deployment, it should pass three gates — not as a compliance checklist, but as a forcing function that surfaces structural gaps before they become expensive in production.

Gate 1: Named Workflow Owner. If you cannot name a specific individual in your organization who owns the clinical workflow this tool operates in, you are not ready to scale. Not the vendor relationship. Not the IT configuration. The actual clinical work, on the floor, every shift.

Gate 2: 30-Day Leading Indicators. Require the vendor to specify three metrics their tool moves within 30 days, using data your organization already collects. If they need to build a custom dashboard to demonstrate ROI, that is a red flag — not a technical gap. It means the tool was not designed with your operational context in mind.

Gate 3: Resistant Unit Validation. Run the pilot in your most skeptical clinical unit, not your most enthusiastic one. If adoption doesn’t reach 60% among resistant users, it won’t survive enterprise scale. A successful pilot in your early-adopter unit is a favorable lab result, not a deployment signal.

For telepharmacy and ambulatory pharmacy operations, three workflow areas are best positioned to clear all three gates in 2026: prior authorization intake, discharge medication reconciliation, and formulary exception routing. These processes are already painful, already instrumented with data, and already producing measurable delay at every handoff point. They don’t make for impressive conference presentations. They are exactly where the operational ROI is concentrated.

Only 12.5% of health system leaders report that autonomous AI has delivered meaningful clinical value to date. The organizations gaining ground aren’t deploying more AI. They’re deploying less AI into better-designed ownership structures.

Executive Takeaway: Three Actions This Quarter

Audit your current AI portfolio for Gate 1 failures. List every AI tool in active deployment. For each one, write the name of the person who owns the clinical workflow — not the vendor relationship, not the IT ticket queue, not “the AI governance committee.” One name. If you can’t fill that field, you’ve located your scaling problem. Fix the ownership model before you expand the license.
